You must demonstrate you have appropriately considered and captured all clinical hazards by submitting a copy of your clinical safety case report and hazard log.
Download the hazard log template for the National Imaging Registry API . The template includes the minimum hazards you must address as part of your clinical safety review. If you identify more than these minimum hazards, add them to your hazard log. You can use our template or create your own.
The clinical safety case report and hazard log must be approved by your responsible Clinical Safety Officer.
