Approved connection agreement

Once all other onboarding stages have been successfully completed and approved, we will initiate the preparation of your Connection Agreement. Upon completion, an email notification will be sent from support.digitalservices@nhs.net to the designated legal signatory, who will be responsible for reviewing and accepting the service terms and conditions outlined in the agreement.

The email notification from support.digitalservices@nhs.net will include a link to the Customer Portal, where your Connection Agreement or Contract(s) can be accessed.

To view and accept the agreement, please navigate to “My Extras” and select “My Connection Agreements”.

The terms and conditions of the Connection Agreement have been published and are available for review here at any time during your onboarding.

Once your contract is marked as “Active” and includes a confirmed start date, we will approve this section by assigning a unique reference number in the format CNTRXXXXXXX (CNTR followed by a seven-digit number).

You can provide a group mailbox as email address. The connection agreement email notification can be delivered to a group email; however, customer portal login access is for the individual person registered as a contact.

We will issue the email notification containing a link to the customer portal that will display your Connection Agreement/Contract(s), for review and acceptance to your legal signature responsible contact/s.
You must register the above legal signature responsible contacts to enable access to the [customer portal](https://www.support.digitalservices.nhs.uk/csm), contacts can be set up by using User Registration - Customer Support form.

Has the legal signatory responsible contact accepted the connection agreement?

A notification email from support.digitalservices@nhs.net will be sent to the designated legal signatory responsible for accepting the service terms and conditions outlined in the Connection Agreement.

This email will include a link to the Customer Portal, where the Connection Agreement or Contract(s) can be accessed by navigating to My Extras and selecting My Connection Agreements.

Internal NHS England products and services are not required to complete a connection agreement.

The NHS England Live Service Team will approve your Connection Agreement by assigning a unique contract reference number, formatted as CNTR followed by a seven-digit identifier

Not providing the relevant information will increase the time of approval and may result in application rejection.
